How are they determined? Is it based on record or is it always East-Central/West-Wild Card type of thing or something totally different.

Wild card plays the team with the best record, unless they're from the same division. Then the wild card plays the team with the next best record. So basically:

Padres vs Cardinals, Astros vs Braves since the Cardinals and Astros are both in the NL Central.

Or:

Phillies vs Cardinals, Padres vs Braves since the Cardinals and Phillies aren't in the same division.

AL's a little more messy.
