We had the exact thing happen a few years ago to a player who played for our Junior A club here. Don Cherry talked about on the next Coach's Corner and I think it was mentioned on TSN.
Anyways, what turned out was the problem was that the player did not have proper shoulder pads. Rather than wearing the proper-size shoulder pads, he was wearing about 3 sizes too small. A lot of players do this so that they can have more mobility for their arms (in shooting, etc) instead of big, bulky shoulder pads. It's too bad, but these things happen as a result.
This is a sad story, I'll be thinking of the family, the organization, and the hockey community there --- because we had the same incident here. RIP
