Don Ashby and Scott Garland, both former Leafs centres who had moved on to different teams, were killed in car accidents a couple of years apart in 1979 and 1981.

Jeff Batters, who played briefly for the Blues, was killed in a car accident in the off-season in 1996.

Link Gaetz received a serious head injury in an accident that ended his NHL career, although he's bounced around minor and senior leagues since then.

Trent Kresse, Scott Kruger, Chris Mantyka, and Brent Ruff of the WHL's Swift Current Broncos were killed when the team bus crashed in 1986.

Baz Bastien, the Penguins' GM, was killed in an accident in 1983.

I seem to remember Floyd Smith, who played for a few teams, being involved in a car accident either while or immediately after he was the assistant coach in Toronto, but can't find details of it anywhere.
