Ottawa still uses the same template, but you gotta think they'll be making their current 3rds their primary jerseys and going to create a complimenting home version.
I guess Vancouver does too, but I wouldn't be surprised to see them change theirs either. (back to the rink logo?)
there are probably other teams that still use them, but im drawing a blank.
