Technically, the IOC doesn't rank countries at all and have consistently reiterated that tabulating gold medals is completely arbitrary.

I'm pretty sure the rankings they release today owe themselves to media pressure back in the 90s (Atlanta?), and before then the IOC didn't even keep count of total medals won.
