Sorry to bring up football again into discussion, but in December 2004, halfway through the season, Sparta Praha fired their coach Straka, while being top of league, with comfortable 9 pts lead in hand. Reason was that Sparta failed to make it out of CLs Group Stage.

At the end of season (some 15 game days later), Sparta not only blew their 9 pts lead, but even gained another 5 pts loss on the champ from Ostrava. At the same time, they managed to qualify into CLs group stage single one time since then.
Jupp Heynckes was fired as Real Madrid head coach immediately after winning the Champions League in 1998 because the team played not nice enough lol.
