It's not constant noise for most teams, and it's a reaction to what's going on. The most common time for my team to chant is when the ball is out of play, especially when a corner or free kick has been awarded.

Absolutely. It's fake, too. Cheering should be natural, the way you're describing. Make noise when something incites you to make a noise
