There is one of these over on MSH, so i thought we should post ours here. We have a lot of beer leaguers on here now so I'm sure there are some creative ones out there.
For the home jersey my teams are based off of the Wild's home reds for the jersey and the Wild's third jersey workmark.
For the away's we used the antique white & red All Star Jersey from 04 in Minnesota, with the same wordmark.

Malibooboo and 99 Problems those look great! Malabooboo I think it is very fitting that your home jerseys are "the whites." I'm sure you never heard that one before.
Here are the backs of our jerseys that I originally tried to post before. It gives you and idea of the detailing and color on our numbers. Thanks again to rk5n for sorting me out.
Thanks! I actually created the logo based off Chernabog from Disney's Fantasia. Never worked in Illustrator before so I got the 30 day demo and figured out how to use it

Thanks again. The idea was mine, but the logo was actually created by a guy on my team who was a little more talented with creating art on the computer. The logo presented a challenge to both of the companies we used for home and away b/c of the fact that it was all lettering and we wanted it on an angle
The homes were done by icejerseys.com. They were a bit pricey, but I guess that's what happens when you get everything fully embroidered as opposed to screen printed.
The aways were done by two companies. We got the actual jerseys from a place called blankhockeyjerseys.com. Great customer service and a massive selection. Really nice quality jersey too. We then had them embroidered by a place called Swiss Craft who does custom embroidery in IL. There was a little more legwork done on this one but the final product ended up being about $15 cheaper per jersey.
A few guys on my team were worried about the "cream" color, but when you see them on the ice with the full uni, they look great. Since I've now gone a little crazy posting pics, here is the view of the full setup:

Thanks Jason. During the Winter Classic I actually heard Joe And Russ talking about your guys getting new ones and having them potentially with a Flyers theme.
I also gave them the info for where we got ours done at.
