I've used www.icejerseys.com for a single custom order a little over a year ago. These guys are top notch and I would highly recommend them. You can order a single custom (that's what I did); you can even have them do lettering on the front (I got my brother's team name on the front). There are a number of teams that have used them in the beer league that I play in.

There are some crazy jerseys when you click on the Other Teams link. I might have to order a Fire Skulls jersey or something like that just because they are so different from what I see at my local rink.
