I have been to a few:

NHL
Calgary - Pengrowth Saddledome (Calgary Flames)
Edmonton - Rexall Place (Edmonton Oilers)
Los Angeles - Staples Center (Los Angeles Kings)
Toronto - Air Canada Centre (technically not for a live game, but was at the NHL draft and watched a Leafs playoff game on the big screen) (Toronto Maple Leafs)

WHL
Moose Jaw - Moose Jaw Civic Centre (the Crushed Can) (Moose Jaw Warriors)
Regina - Brandt Centre (Regina Pats)
Saskatoon - Credit Union Centre (Saskatoon Blades/U of S Huskies/World Juniors/NHL Preseason/Canada Cup)

SJHL
Saskatoon - Harold Latrace Arena (Saskatoon Titans)

I've also been to a few venues that have hosted hockey, but not for hockey games:

NHL
Edmonton - Commonwealth Stadium (2003 Heritage Classic)
Montreal - Bell Centre (Montreal Canadiens)
Montreal - Montreal Forum (though it was after it was turned into a movie theatre) (Montreal Canadiens)
St. Petersburg - Tropicana Field (was called the Thunderdome when the Lightning played there)
Winnipeg - MTS Centre (Winnipeg Jets)

AHL
Hamilton - Copps Coliseum (Hamilton Bulldogs)

ECHL
Orlando - Orlando Arena (Orlando Solar Bears)

AJHL
Lloydminster - Centennial Civic Centre (Lloydminster Bobcats)

WHL
Saskatoon - Saskatoon Arena (Saskatoon Blades)

My goal is to visit every NHL arena and see my Kings play there.

NHL
Nassau Veterans Memorial Coliseum (NY Islanders)
First Niagara Center (when it was the HSBC Arena) (Buffalo Sabres)

AHL
Blue Cross Arena at the War Memorial (Rochester Americans)
Onondaga War Memorial Arena (Syracuse Crunch)

WHL
KeyArena (former home of the Seattle Thunderbirds)
Comcast Arena (Everett Silvertips)

ECHL
US Bank Arena (Cincinnati Cyclones)

College
Wilson Ice Arena (Marquette University, Milwaukee, WI)
Ice Arena (Brockport Golden Eagles, SUNY Brockport, NY)

Of the ones I attended a hockey game:

Best:
First Niagara Center, Comcast Arena, Blue Cross Arena

Worst:

Nassau Coliseum (This is despite attending Isles games during their Stanley Cup years. Other arenas I've visited, at least from the outside, are better designed and located in more convenient locations than Nassau Coliseum.)

Key Arena (All arena should accommodate a hockey rink for the entire space. Because of former Seattle SuperSonics management and ill-advised decisions by the then-Seattle city council in the 1990s, the rebuilt became a basketball-focused arena with hockey games configured in a horseshoe. I had no problem with the dedicated Thunderbird fans and any road team fans, which made the arena environment less frustrating.)

Expanding on the question further,
On a guided tour, not for a game:
Centre Bell (Montréal)

Attended a game at the venue but not for hockey:
Madison Square Garden
MECCA Arena (Milwaukee Admirals when the team was in the IHL)

Seen venue in person, did not attended a hockey game:
HP Pavilion, San Jose
Pepsi Center, Denver
TD Garden, Boston
Rogers Arena, Vancouver
Air Canada Centre, Toronto
Maple Leaf Gardens, Toronto
Copps Coliseum (Hamilton Bulldogs)
Rose Garden and Veterans Memorial Coliseum (WHL Portland Winterhawks)
John Labatt Centre (now Budweiser Gardens) (OHL London Knights)
Save-On-Foods Memorial Centre (Former ECHL Victoria Salmon Kings, currently WHL Victoria Royals)
ShoWare Center, Kent, WA (Current home of WHL Seattle Thunderbirds)
(Among others venues I can remember.)

Hope to attend an NHL game in the future:
Vancouver
Edmonton
Calgary
Denver
San Jose
Seattle, WA (Fingers crossed.)
Brooklyn, NY (I'm an Islander fan from their Stanley Cup years.)

Enjoyed watching the few games I attended at the former HSBC Arena. My favorite memory: standing in line buying some food and carrying on a conversation with another attendee who bought tickets to the Sabres games because he could not get tickets to the Leafs.

I do appreciate the AHL/WHL teams for what they are able to do for their local communities if the sport is marketed properly. For an area that has had a WHL team for 10 seasons, Everett's Comcast Arena can still average nearly 7,000 for a weekend game. The arena's hockey capacity is around 8100.

Sorry for the extended post. Visiting various venues brought back some very nice memories.

With the home viewing experience getting better and better, I find that my motivation to get out to live events is getting smaller and smaller.

I'd have to be handed tickets or get hefty discounts to consider it at this point, especially with the market price for NHL tickets around these parts.

For me:
Save-On-Foods Memorial Centre
Key Arena
Pepsi Colisee
Barclays Center (Got pictures of it under construction, stayed in a hotel nearby.)
IZOD Center (From the very top of MetLife Stadium.)

Would like to see a game at:
Saddledome (Always wanted to see a Canucks road game here some day.)
New Seattle arena (hopefully)
Air Canada Centre
Scotiabank Place
Bell Centre (I was there for a preseason game, would like to see a regular one if I ever return, especially if playing the Canucks.)
MTS Centre
United Center
Barclays Center (My brother is an Isles fan and we have been to New York for games a couple times, so this is a certainty in a number of years. And the travel time to the arena will be cut substantially next time.)
Bridgestone Arena (Seems like a great atmosphere.)
Consol Energy Center
Xcel Energy Center

Curious what did people who went think of Commonwealth stadium. We know rexall is a dump but how does the outdoor arena fair?
