I'm selling this one for $100 (Mens Large)

Fact. This jersey style was "worn only once by Team Canada", and it was only in Ottawa for the 2004 World Cup of Hockey. That's how rare they are!

I don't think it was a screw up, I think he intentionally had it made up that way because you can buy blank crest-less jerseys. The factories wouldn't have any of the old 2D patches available.

This.

Look at how high the crest is on the front of the jersey. It's that high up, because while you can buy blank jerseys of that style, they have the Reebok wordmark right below the neckline of the jersey. Here's a blank Avs jersey as an example:


He's hiding the "Reebok" logo on the front by intentionally putting the old 2D crest way too high. This guy has clearly bought a blank, bought the patches online, and just did it himself. I've done it before too (I sewed a Whalers logo to a blank Hurricanes jersey for a friend as a joke gift) it's fun sometimes just to mess around with jerseys.
