This has been one of those things that I've wanted to find since this thread started years ago, and I'm convinced that it hasn't happened. I think I've gone through every instance from the 70s and 80s where a player had 2 fights in a game (from dropyourgloves), and couldn't any with 2 goals and 2 assists in the same game. There were so many times where a player like Hodge, O'Reilly or Bridgman would be just one assist short.
It's possible it happened earlier than that, but doubtful considering how scarce assists were in the early days, and how low the PM totals were for the top scorers in the 50s and 60s. And it's also always possible that there may be a missing fight not on DYG, or a missing assist not on the HSP, but I've given up trying to find it.
I wouldn't be surprised if it happened in the WHA though. And the junior leagues are also a possibility.
