Lost USA Hockey Insurance Card

I forgot my username and password too for USA Hockey. I'm playing a summer league and I need it for registration and I do NOT want to pay another $30 to get it.

Anybody know what to do? Thanks!

not a scam.... For uninsured, under insured or others, it can save you a decent amount of coin.

For those who have not met deductibles, have no private insurance or simply have remaining balance due after insurance pay outs, USA Hockeys coverage takes care of these expenses.

With out this very inexpensive coverage I could only imagine how many associations simply wouldnt be around or how much the fess to play would skyrocket.
